Guide: Reimbursement for a Driver’s Medical Exam
Pacific Blue Cross will reimburse you for the fee your doctor charges to complete the mandatory driver’s medical exam, if the claim is done properly. Unfortunately, medical exam expense claims cannot be submitted online or via the PBC app. You must submit a completed extended health claim form.
Be sure to write the following on the form:
“Driver’s medical exam required to maintain class 2 (or class 4 for CTS operators) driver’s license for employment.”
In addition to the claim form, please submit an original paid receipt that indicates the following:
- Patient’s name
- Date of service
- Description of service
- Amount paid
Claim forms are available on the Pacific Blue Cross website (www.pac.bluecross.ca) and should also be available in the depots.
Mail both the form and the receipt to Pacific Blue Cross, or deliver them to the dropbox at the Pacific Blue Cross office in Burnaby:
4250 Canada Way, Burnaby, BC V5G 4W6
Keep copies of both for your own records as well.
